Abstract: 

All along the history of medicine, Case reports have always been a rich resource for teaching and research. Nowadays Case reports are published in many medical journals. Publishing these reports often sounds easy, but if not provided in a systematic approach, usually have little value for other researchers and the rejection rate can be extremely high. As the number of submitted reports to medical journals are increasing, the author should know how he/she can select and report the Case in order to persuade editors to accept the report. Recently, the Case reports have been considered as a source of reliable evidence in evidence based medicine. Interestingly, evidence based reporting of Cases has come to the center of attention of modern article writers.The Case must be chosen properly and presented in a brief but informative way. Both positive and negative outstanding results related to the Case must be noted. Discussing characteristics and unique aspects of the Case is of paramount importance. The author should try to cite some important references in the discussion section of the article. In the present review, We tried to describe the way in which a clinician  can write and publish a classical Case report.

Abstract: 

About one century ago, Garrot defined the first disease of inbron error of metabolism. Many diseases have been classified in this category. Organic acidemia with multiple manifestation is one of them. A 12 year old boy with several episodes of metabolic acidosis and one attack with loss of conciousness, but with lack of skin manifestation was admitted to the clinic. His diagnosis as methyl malonic acidemia has definitly been confirmed by sending a sample aboard. Although such diagnosis is difficult to make in Iran, but our information about such Case will be helpful in correct diagnosis of any Cases with unexplained metabolic acidosis and developmental delay.

Abstract: 

2Introduction: The umbilical cord is often encircled around different parts of the fetus, usually the neck, which is a common phenomenon in the delivery room. These entanglements may form early in pregnancy, although there is no definitive evidence on the exact time of their formation. The aim of this study was to investigate a fetus with four turns of the umbilical cord around the neck as a Case report. Case introduction: The studied patient with G1P1, aged 32 years, referred to Sarem women's hospital due to slowing down of the fetal movements. After performing the NST test and detecting a decrease in the fetal heart rate in the 40th week of pregnancy, the mother was hospitalized for an emergency cesarean section. The baby had four umbilical cords around his neck. The baby's Apgar score was 9/10 and seemed healthy. Finally, the mother and the baby were discharged in perfect health on the date after the conditions were stabilized and there was no complication. Conclusion: Rotation of the umbilical cord around the neck may be associated with an increased risk of adverse perinatal outcomes. In this study, the infant was born by emergency cesarean section at 40 weeks of gestation due to danger of death following spontaneous delivery. Continuous routine monitoring of laboring patients using cardiotocography can be the best way to achieve a good result.

Abstract: 

مقدمه و هدف: در این مقاله دو روش آموزشی تحت عناوین Case Study ,Case Method به عنوان تکنیکهای آموزشی مورد بررسی و نقد قرار گرفته و تفاوتها و شباهتهای هر کدام به طور جداگانه بررسی شده است و نکات کاربردی هر روش در آموزش پرستاری مورد بحث قرار گرفته است.مرور مطالعات: در این مقاله ابتدا تعاریف دو نوع متد آموزشی ارایه گردیده و سپس موارد استفاده از هر متد به طور جداگانه بحث شده است، و با توجه به ماهیت آموزش پرستاری ایران پیشنهادات کاربردی در این زمینه ارایه شده است. Case Method در گروههای آموزشی کوچکتر که مشاهدات ذهنی کمتری دارند و در ابتدای تجربه می باشند استفاده می شود. ولی Case Study در گروههای آموزشی بزرگتر که مشاهدات ذهنی بیشتری دارند و قدرت تجزیه و ترکیب و رشد بحث در آنها بیشتر می باشد استفاده می شود. از ویژگیهای مهم آنها می توان به افزایش قدرت تصمیم گیری افراد در موقعیتهای مختلف، لذتبخش تر کردن آموزش و علاقمند کردن به امر تدریس و ... نام برد.بحث و نتیجه گیری: با توجه به یافته های پژوهش و با توجه به محتوی برنامه های آموزشی پرستاری، محقق استفاده از روشهای Case Study ,Case Method را برای دانشجویان پرستاری توصیه می نماید. زیرا بهترین آموزش یادگیرنده ها زمانی مطرح می باشد که دانش هماهنگ و متنوع مهارتهای آموزشی با تجربیات در کنار هم می باشد.

Abstract: 

Introduction: Urinomas are rare and uncommon lesions that occur in the ureter or around the kidney. The aim of this study was to report a Case of a rare ureteral urinoma. Case Presentation: The patient was a 69-year-old woman who referred to a medical center complaining of left-side pain. The results of the patient's tests showed low creatinine and higher than normal urea. According to the abdominal and pelvic ultrasound findings, ureteral urinoma was diagnosed. The patient was discharged from the hospital after taking the prescribed medicines on November 26, 2022. Conclusion: The findings of this study can help the treatment staff to consider the possibility of diagnosing urinoma in patients who present with symptoms of fatigue and flank pain. Therefore, being aware of the symptoms of this disease will help diagnose these patients faster and prevent the disease from worsening.

Abstract: 

Introduction: Stoma significantly influences the biological and psychosocial dimensions of individuals, creating an effective and integrated demand for self-care. The Orem Self-Care Deficit Nursing Theory provides a conceptual framework for the nursing process in this regard. Therefore, this study aims to impact the care process based on the Orem Self-Care Deficit Nursing Theory on individuals living with a stoma. Methodology: In this Case report, the nursing process for an elderly individual with a stoma, who referred to the Bessat Healthcare Center in Hamedan in the year 1402, was presented based on the Orem Self-Care Deficit Nursing Theory. Utilizing the Orem assessment form and employing examination, interview, and observation methods, data were collected. According to the universal, developmental, and health-deviation self-care needs, nursing diagnoses were determined, followed by planning and goal setting. The type of nursing system was also defined based on these diagnoses and implemented accordingly. Finally, nursing interventions focused on enhancing self-care were carried out, and progress and goals were reassessed and evaluated. findings: Based on understanding the self-care needs of the patient, 7 nursing diagnoses were determined. Ultimately, implementing the nursing process according to the Orem Nursing Self-Care Deficit Theory led to an improvement in the quality of nursing services along with an increase in the patient's self-care performance regarding issues arising from their illness. This improvement was observed in enhancing the patient's self-care ability in terms of increased self-confidence and self-esteem, reduced risk of infection, promotion of skin integrity, increased sense of independence in the patient, enhancement and maintenance of social relationships, fluid and electrolyte balance, and increased appetite. Conclusion: The Orem Nursing Self-Care Deficit Theory proved effective in identifying and comprehending the comprehensive needs of the patient, providing substantial assistance. By involving the patient in self-care, it facilitated the achievement of care goals.Therefore, it is recommended to use Orem's self-care deficiency nursing theory in order to fully identify the needs of patients.
